By checking this box I acknowledge that all equipment on this job site is my responsibility to look after and maintain. I have checked each piece of equipment daily and have greased, cleaned, adjusted each piece as needed (at least once per week). If there are any problems with the equipment, I have notified in writing to the office for it to be repaired and have taken the equipment out of service until the repairs are complete.
